Player Forms

Player  Medical Release Form
Must be turned into your player's coach prior to the first practice

School Enrollment Form
Please use this to verify your proof of residency, if your player attends a Redmond School District school (except Tumalo). Please see RLL Boundary Map for more information on this.

Accident Form
If any player, coach or umpire is injured during a RLL sanctioned activity (practice, games, clinics), this form must be filled out and turned into RLL's Safety Officer as soon as possible. If you cannot reach the Safety Officer, please touch base with your coach, or email us here.

Coaching Tools, Links, and Such

**Requirement to Coach for RLL** -   HEADS UP Online Training
Each year, all Coaches must complete the online HEADS UP to Youth Sports training through the CDC website. It's highly recommended that you save your completed certificate to your phone or computer, in case you need to resubmit it later.  Completed Certificates need to be emailed to [email protected].

**Requirement to Coach for RLL** - Abuse Awareness Training
All coaches must complete the Abuse Awareness Training certificate. Here is how to do that:
1. Click the Link that says 'Abuse Awareness Training'.
2. Select Education from the menu
3. Select Courses
4. Sign up or in
5. Select abuse awareness for adults
